本校大一新生因故不能出席大學入門各種課業及活動者，辦理請假申請。
Freshman Students who are unable to attend this Orientation can apply for a leave.
※本假單僅限大學入門使用，學期中請假請依東海大學學生請假規則辦理。
This leave request form is only for the Freshman Orientation. Please follow the Tunghai University Student Leave Rules to apply for a leave during the semester.
※本申請表之個人資料蒐集、處理及利用，僅供本校大學入門請假相關作業使用，並遵守個人資料保護法相關規定。
According to The Personal Data Protection Act, the collection, processing and using of personal data in this form will be used for the leave request for freshman orientatiom related work only.
